The PSHS System Board of Trustees in its meeting dated June 20, 2018 resolved to approve the Revised PSHS Scholarship Agreement and the updated List of Approved Science and Technology Courses as per BOT Resolution No. 2018-06-49 to be implemented effective SY 2018-2019.

In connection to this, the PSHS-CMC Scholars and Parents shall execute the Revised PSHS Scholarship Agreement and the updated List of Approved Science and Technology Courses superseding the old. Download this form and print it in A4 size of bond paper into four (4) sets. Fill-up the 4 sets of Revised Scholarship Agreement form and sign at the side of every page except the last page. Signature of both scholar and parents must be original. Place these 4 sets of signed Revised PSHS Scholarship Agreement in a brown envelop and submit them to the Registrar for the signature of Campus Director on or before August 1, 2018 then, these will be returned to you for notary public. Once notarized, submit immediately the 4 sets of Revised Scholarship Agreement to the Registrar.
